Terms & Conditions SERVICE AGREEMENT: I understand & agree that the service is continuous from year to year until I cancel in writing which I may do at any time. I also understand that Healthy Lawn’s Programs work best with a lawn that is mowed at least 3 inches in height & that anything less will adversely affect the results. Program/Service Selection, Payment and Terms: Your Program/Service will start with the number of applications shown on the front of this estimate form or if you are online the services you have selected. After each application an invoice will be left for prompt payment. (Payment is due upon receipt of invoice). We now accept credit and debit cards online at www.myhealthylawn.com. The Products we use: A list of the products we might use is included with this estimate/contract or dropped of with the service invoice at the time of application in compliance with Article 33-section 0905.5A of the Environmental Conservation Law. If during the course of the year we decide to use another Product you will be sent or given a copy of the label prior to its use on your property. All Products we use are registered with the EPA, are biodegradable & organic in nature. We use the latest, most up to the date Products available. Fertilizers are organic based. Once an application is made, in about 2 hours it will be dry and safe for you to enter the property. Do not enter until the turf is completely dry (in the case of a liquid application). It will be your responsibility to water in the application when required. (We would notify on your drop off invoice if this was necessary) Notifications of Applications and Waiver: Our applications are scheduled in the following date ranges: (Subject to weather conditions). Lawn visit 4-(7-1-23 to 7-21-23) alternate (7-22-23) to 8-11-23) Lawn visit 5-(8-1-23 to 8-21-23) alternate (8-22-23 to 9-11-23) Lawn visit 6- (9-1-23 to 9-21-23) alternate (9-22-23 to 10-12-23) Grub Control is applied (6-1-23 to 6-21-23) alternate (6-22-23 to 7-12-23) Tree/shrub visit 1-(3-15-23 to 4-1-23) Tree/shrub visit 2-(4-1-23 to 4-21-23 alternate 4-22-23 to 4-30-23) Tree/shrub visit 3-(5-1-23 to 5-21-23 alternate 5-22-23 to 7-31-23) Tree/shrub visit 6-(8-1-23 to 8-21-23 alternate (8-22-23 to 8-30-23) Tree/shrub visit 7-(9-1-23 to 9-21-23) alternate (9-22-23 to 9-30-23) “The property owner or owner’s agent may request the specific date or dates of the application(s) to be provided and, if so requested, the pesticide applicator or business must inform of the specific dates and include that date or dates in the contract. As a customer you have the right to be notified of alternate pesticide application dates if the pesticide cannot be applied on the indicated dates on this contract. However, you are waiving this right by Accepting this agreement. Late Payments: invoices which are not paid in 30 days of invoice, will be considered past due. We reserve the right after 90 days to cancel service at any time and void all guarantees. We also at our discretion can start to charge interest at the rate of 1.5% per month after 90 days. A $30.00 service charge will be charged for returned checks. Service will be resumed when balance is paid in full. If collection action is necessary, all collection fees, attorney fees and court costs will be added to the amount owed. If you received any discounts and your account is cancelled you will loose any discounts applied for work done to date. Continuation of Agreement and Cancellation Procedures: I would like this contract to continue from year to year based on this agreement until I cancel in writing which I may do so at any time. All guarantees are voided when service is cancelled. Guarantee: Healthy Lawn’s goal is to give the best service. If at any time you have a question or complaint please feel free to call. Again, please call. We will service you with free re-sprays, service calls and additional fertilizer applications as needed according to the limitations as set forth on the front of this form until you are happy. Please note: The grub prevention guarantee is voided unless you notify Healthy Lawn Inc, immediately when damage is present. We will not honor any Grub Prevention guarantee when we are notified the following year or after the grubs have left the root zone and gone deeper into the soil. Harmful if swallowed, inhaled or absorbed through skin, eyes or clothing. Avoid breathing vapor or spray mist. 4. ROUNDUP PRO-EPA REG# 524-529 (HERBICIDE) Caution Keep out of reach of children. Hot line # 1-800-314-4000. Causes mild eye irritation. 5. MERIT 75 WSP-EPA REG # 3125-439 (INSECTICIDE) Caution Keep out of reach of children. Hot line # 1-800-414-0244. Harmful if swallowed, inhaled or absorbed thru the skin. Avoid contact with eyes, skin or clothing. Avoid breathing dust or vapor. Keep children and pets off area until spray is dry. I have read the above Service Agreement and by signing this agreement, I agree to its terms and conditions & I understand that the service is continuous from year to year until I cancel in writing.
